We have designed some [Design Template] for form for free download, please select from the following template categories.
[White] [Yellow] [Orange] [Red] [Purple] [Blue] [Green] [Brown] [Black]How to implement template into your webpage :
1) Create your form by following the instruction as per the [readme] folder.
2) Copy the file from zzmain/amain/a/index1.php to the [destination_folder] and :
2.1) change line13 $selfuri1a='zzmain/amain/a/'; to the [destination_folder]
2.2) change line15 $selffile='index1.php'; to the name of the file.
3) Add membership by following [Add membership.html] at the [readme] folder, If you want to let the public to view this file, omit step 1) and step 3), assign this page for step 2) zmem12200*(f02)=public, So now this form can be view by public
4) Now download the chosen template by copying the code and save as a CSS file. Include the template downloaded into field [csslink] in table [zpg].
